A 32-year-old man was sent by his sister, who feels sorry for him. In two weeks, he is getting married, but he has a pigmentation disorder. For the past two years, his skin has not tanned. His arms are brown up to the line of his shirt, but his torso remains pale. Last year on vacation in Italy, he didn’t get sunburned but also didn’t tan. The skin in his elbow creases is often irritated, itchy, and red, which also started two years ago.
As a child, he had eczema and occasionally flaky skin on his face. Otherwise, he is healthy and has never had any illnesses. A skin scratch test is negative. He works as a tiler, and his skin sometimes cracks from the cement. He always worked fixed hours and didn’t want to request time off. He came to the consultation because he happened to finish work by 6 p.m. on the day his sister called my practice. He sleeps well and exercises regularly. His life has been the same for ten years: his house, his relationship, and his job at a small company. The job started as a holiday job, and he liked it so much that he stayed. He used to tan normally.
He is a calm, open young man, with a reserved attitude. He connects well with others and is sensitive to his environment. When he thinks, his gaze doesn’t turn inward or far away but remains just in front of him. This type of nonverbal expression is often seen in Silver series. Otherwise, there are no significant indicators.
Analysis
There were no clues about the cause or a biological explanation, so the search was based on association.
What is commonly used to bleach the skin? Citric acid, lemon juice. One possibility from this perspective was the remedy Citricum-acidum, but the "acidum" aspect, as known in homeopathy, was not recognizable.
