Case by Jan Scholten.
Case: boy, 7 years old, autism. He is autistic. His speech is delayed, but he uses a mix of languages, Polish, Dutch and English. He uses about 200 words. He understands more than he can say. His toilet training has been very difficult: he is bedwetting, and pees wherever he is. He defecates in his nappy.
His muscles are weak. Finer movements, like drawing and writing are difficult. Walking is no problem, but he knocks against everything. He is restless, walking around in the consulting room and taking everything form the shelves. He doesn’t like to be limited and when he is limited he knocks down a glass jug of water and it brakes into pieces on the ground. He does the same with two pieces of pottery and looks defiant when doing it, to see how others react. He creates chaos everywhere.
He has grinding of teeth. His tongue is coated white.
During his pregnancy his mother had diabetes, obesity, haemorrhages from her uterus, hypertension and pre-ecclampsy. Her vision was blurred at the end of the pregnancy. She felt stressed and was anxious of losing her baby as she had a a miscarriage in the former pregnancy. The labour was induced two weeks before the indicated time due to the pre-ecclampsy. His mother felt sad and locked, as she was not allowed to move or walk. The labour lasted long.
As a baby he was sleepless and his other had to walk around with him on her arms to let him sleep.
At school he is doing everything the teacher wants. He sits in the circle and adapts. He likes other children and they run around together or bicycle.
He is not shy. He is always active, busy. He must move or even run, even at dinner. He touches everything, opens all drawers and cupboards. He has a tendency of throwing things away, and demolish them, especially when he is angry, when he does not get what he wants like a mobile phone. Then he can be explosive. He has an aversion of his head and face being washed, and his nails and hair being cut. He makes noises, sometimes he is screaming. He can laugh without apparent reason. His favourite things are animals, farms, cars, building and movies for children. He can be obsessive, putting things and toys in a precise row.
He can have short periods of freezing, staring and just standing for half a minute.
He has good contact with his parents, especially his mother. He can cry when his mother leaves for work. On the other hand he seems to be indifferent when she come back. There is alcohol abuse in the family. His father and grandfather drinks early in the day. His mother feels alone with all problems, feels bizarre. His mother had bad contact with her sister.
