Case by Jan Scholten.
Case, boy, 11, autism. He has obsessive thoughts that make him anxious. They are chains of thoughts that make him think something bad could happen. For example, he thinks he could be punished if he puts on his socks or if he pulls a blanket over himself. He feels as if there are two persons inside him, a good one and a bad one. He then thinks: wait until the thought is over or I'm going to stop the thoughts. Thoughts he gets for example are to pound on a car with a hammer. He has childish questions like what will happen if he goes outside. He is indecisive because he is afraid of making the wrong decisions. Then he may start crying because he thinks both are right or both are wrong. He doubts whether he should take a path to the left or right. He tends to name his mother objects such as a piece of clothing or a pig. He may think: “I am the door, I am the pizza”. He makes obsessive hand movements. His thoughts tell him that his sister is dead. He smells the odour of a dead body. He does enumerations like: "Three things, two of which fit and one of which doesn't. And the one that doesn't fit turns into the one that does. For example, a flower, a plant, a towel. Or some fairy tale characters, if one doesn't fit, they change into another".
He likes to ride his bike and play soccer, especially in the spring and summer. In the winter he gets bored quickly and then goes to look for food or play a phone to play computer games. He likes computer games with shooting and killing. He is very interested in fire and burning things to ashes.
He is overweight. He often has diarrhoea, especially when he drinks milk. He has had type one diabetes for three years and uses insulin for it. He has to pee a lot even though he drinks normally. During his pregnancy his mother drank beer regularly. She did this because of tensions with her partner as they disagreed about having a child and how to raise it. Her partner abused alcohol and drugs. The delivery was on time, natural and without complications.
He is doing well competing in school and has average scores. His memory is good. But he feels out of place, blocked. He feels like his teacher is a ghost. His handwriting is terrible, almost illegible.
He dislikes interacting with other children and prefers to play by himself. Gradually, he has gained some friends at school and is beginning to be accepted in the classroom. But after school he wants to be alone and no contact with friends. He has good contact with a cousin. He gets irritated when children repeat questions. In general, he is friendly and not aggressive. He gets anxious when he has to wait, for example, for his cousin.
He has good contact with his mother. His parents divorced when he was a year old because things didn't work out since his mother was pregnant with him. His father kept going out, partying and leaving his son at home alone. He sees his father once a year in the vacation. He does not have good contact with his stepfather, who ignores him.
He is awkward and careless with his half-sister. He presses on her, lies down on her and does not respond to her crying. He does not see that his sister finds this terrible.

He accepts his parents and has gained admiration for them. He does not talk to him about his school friends. His facial expression has become normal. He needs more compliments and supports.
