Case by Jan Scholten.
Boy, 4 years old, autism. From the age of two, he makes difficult contact and does not look at people. He talks little and also asks little. His speech is variable, at times better and then not good at all. He doesn't use many words, but he understands almost everything. He usually points to what he wants.
The pregnancy went well until the last two months. Then the gynaecologist recorded a lack of oxygen in the umbilical cord. His mother was stressed and sad because of her father's illness and the death of an uncle. In childbirth, she received epidural anaesthesia but was still in a lot of pain. Labour was difficult and he had a purple head after birth. As a baby, he hardly cried or talked. Potty training went quite normally.
He has very good contact with his mother and accepts only her. He has little with his father and does not make eye contact with him. His older brother tends to bully him, especially when he gets attention.
He attends a normal school. Sometimes he feels like school, but sometimes not. His concentration is very poor and does not feel like doing anything at school. He has a good memory.
He gets along very well with other children. He is also invited to other children's parties and he likes going to them. He likes to play with other children. He likes to go to football, but he does not participate. He stands there clapping and laughing at what is happening. He often sits separately and then plays alone. He tends to huddle together in a foetal position. He likes to help his parents in the kitchen, making pasta for example.
He is quick to get angry, over trifles, but that also goes away quickly. He gets angry if he doesn't get his way or when his parents take a different route than usual. When he is angry he can bang his head on the floor or against the door. He has a sense of humour and is easily cheerful, but sometimes quickly gloomy.
He regularly has ear infections. Then he is a bit sick, with asthmatic symptoms. He has frequent colds with coughs, especially in autumn.
