They have a strong suspiciousness, a feeling that (part of) the world is against them and tries to harm them. But it is not open; it is all hidden in the dark. They have the feeling that there is a conspiracy going on against them. Their family or the police cannot be trusted either as they are part of the conspiracy. It can be that it is only one person who tries to come and get them, like the situation of an ex-husband who is stalking, phoning them without telling his name, or watching them in the evening or trying to break into their house. As things are hidden it is very difficult to defend yourself against it. When they talk about it to other people or the police, they think that their stories are exaggerated or complete fancies. As a consequence they even feel more vulnerable, unprotected and out of contact. In order to get protection they start to manipulate people, tell their stories a bit differently, more or less exaggerated.
They themselves get into a state that they themselves do not know anymore what to believe. They tell their stories with a laugh, a kind of hysterical quality. The consequence is that others do not know what to believe of their stories. So the paranoia grows.
