Male, 18 years oldHe has a two-year history of profound emotional numbness and depression. His hobbies included drawing and reading history books, although over time he had lost much of the enjoyment he once found in these activities. When asked about the problems he was facing in life, he replied that everything felt "normal and ordinary." Nothing brought him joy anymore. Every thought seemed ordinary, even the thought of death, and he was unable to connect emotionally with his friends. He explained that everything happening around him felt completely predictable and nothing could surprise or move him. Eventually this emotional detachment spread into all of his relationships.
The worst experiences in his life had always been the breakups with his girlfriends. Each of them had left him, although he never understood the reasons. He rated his quality of life at only 4%. When asked why he had wanted to end his life, he replied that he simply could not find any reason to continue living.
His mother explained that the major turning point came when he decided that he wanted to transition to female. He believed that becoming a woman would finally make him happy and complete. He repeatedly stated that he felt uncomfortable in his male body because women could dress beautifully and look attractive. According to several therapists, however, his underlying need appeared to be a desire to become the center of attention, to be admired. He had always been highly artistic, playing the piano and attending drama classes, but since becoming preoccupied with gender transition he had gradually abandoned all of these interests and no longer enjoyed anything.
