7. Case of Acid refluxPresenting Complaints: A 49-year-old male presented with gastrointestinal complaints: acidity with loud, offensive burps (6–7 times, < stretching), burning in chest, loss of taste, abdominal bloating, minimal flatulence, and no time modality.
Past History: Nothing specific.
Family History: Mother and Sister – vitiligo; Father – prostate cancer
Physical Generals: Appetite low, can't tolerate hunger; a foodie, tries out new cuisines; Stools: 2/day; Urine: NAD; Perspiration: profuse, offensive on entire body; Sleep: good, sleeps on the abdomen/side
Mind State: Easily irritated; generally happy-go-lucky and indifferent to others’ opinions. Previously had frequent temper outbursts with shouting (no physical aggression). Sensitive to provocation but now suppresses reactions, believing confrontation worsens it. Gets excited easily. Under stress (notably past financial stress), tends to sleep. Enjoys company, dislikes being alone. Outgoing, loves nature and trekking, with a strong need for stimulation and excitement (paragliding, bungee jumping, scuba diving).
Analysis: The main pattern is stress, reflux, and a strong need for thrill and excitement. The remedy should match this overall pattern, not individual rubrics. A rubric-based selection is not suitable here.
Stress is linked to cortisol from the adrenal glands, while thrill and excitement relate to an adrenaline rush.
Rx:
Adrenalinum 30C B. D. for 2 days
Follow up in 45 days: Acidity complaints much better – 2-3 times per month.
Rx:
Adrenalinum 30C B. D. for 2 days. Repeat in 3 weeks. SL 40 days
Follow up in 3 months: Burning sensation in the chest for the past few days, with frequent eructations, occasional bloating, and continuous loud burping.
Rx: Iris 6C T.D.S. for 10 days followed by Iris 30C B. D. for 3 days. After 3 weeks,
Adrenalinum 30C B. D. for 2 days followed by
Adrenalinum 200C B. D. for 2 days. SL 45 days.
Follow up in 6 months: Overall better
