A 15-month-old girl with sleep problems. She has never slept through the night and has difficulty falling asleep on her own; she needs to be held. She is very restless, and it takes a long time for her to fall asleep. She wakes up very quickly and starts crying. She seems to sleep better in a busy room. In the evening, she is restless and lively. She has a skin fungus on both legs, the soles of her feet are peeling, and her mother also had a skin fungus during pregnancy. She is not a discontented child and needs a lot of exercise. During the day, she is cheerful, needs little sleep, and enjoys herself. She has a very high pain threshold and is feisty and persistent. Her urine has a powerful smell, like ammonia, and the skin of her labia irritated and bloody, sometimes with a rash and blisters. Her urine is normal.
Pregnancy: The mother vomited for five months and had to be hospitalized because she had a lot of amniotic fluid. She felt fine during the last month, and the amniotic fluid decreased. Labor did not start, so she was stripped and had an episiotomy, after which she pulled the child out herself. During a check-up, the baby was found to have a heart defect, small holes, and a thickened heart valve, which will correct itself. The mother was under a lot of stress during the pregnancy. She tearfully explains that a family member was suicidal, and she couldn't let it go. “We were in survival mode for a year after birth. I didn't enjoy the first year. I had wanted to be a mother my whole life, but I like other children more, which I feel very bad about. Of course, I take responsibility. I kind of started to escape.”
AnalysisRubiales: Mental activity and difficulty falling asleep, need for contact, is known from this family. Sleep problems, restlessness, lots of activity, they call her a nag, this is reminiscent of
Cinchona officinalis, and can be extrapolated to the whole family.
There is no physical cause, as the bladder and intestines are fine, and the child is cheerful. With this as base, the section ‘urine, odor like ammonia’ was reviewed, where Cahinca stands out among the 62 possibilities listed. This is the synonym for

Follow-upAfter a week, she is sleeping well, she is more rested, falls asleep more quickly, and tosses and turns less in bed. During the first few weeks, there are still nights when she does not sleep well; these are the days when she does not have a bowel movement. The ringworm on her legs is gone, and the spots on her labia are also disappearing. Her urine now smells fine and is less strong. She wakes up less often during the night. At daycare, they describe her as “so pleasant and so cheerful.” After a month, everything is back to normal. She still stays awake until 9 p.m., but then sleeps through until 7 a.m. She gets colds and stomach flu without any setbacks. She was healthy the coming years.
Comment: As usual, for an analysis of children, you can use the themes during and around the pregnancy and, of course, as here, the child's family situation. Consider the ‘big picture’, everything the child experiences and influences, and choose what is prominent, the predominant factors. The situation during pregnancy and the course of the pregnancy, the feelings and emotions that prevail among the parents or in the family, are always a good starting point for an analysis of a toddler.
