Male, 33 years old, schizophrenia .
Diagnosed with schizophrenia, with two past psychotic episodes. He is currently stabilized on medication (brexpiprazole, venlafaxine, olanzapine), but his main complaint is profound exhaustion, apathy, and lack of motivation. He can sleep through entire days, often until the afternoon (even 4 p.m.), yet still feels unrefreshed. Mornings are particularly difficult—he struggles to get out of bed, feels heavy, and has no drive to start the day. Even when awake, he remains mentally dull and physically drained. He describes losing interest in almost everything he previously cared about and feels he has achieved very little in life.
Psychotic state and background
During psychotic episodes he completely lost connection with reality. He spoke in riddles, perceived hidden meanings and symbolism everywhere, and withdrew into an inner world. There were also obsessive and intrusive aggressive thoughts. The onset was linked to a difficult life period, dissatisfaction at university, social isolation, and especially excessive involvement in spiritual teachings combined with sleep deprivation, which culminated in a breakdown and hospitalization.
His says he has a tendency to escape into spirituality instead of engaging with real-life demands. He describes himself as proud, particularly regarding his knowledge and spiritual understanding, often looking down on others and struggling to form genuine relationships. He had an ideal of connecting only with people on a similar “spiritual level.” At the same time, he has a strong sensitivity and a desire to help others, even seeing his condition partly as a “gift.”
Childhood
Childhood was outwardly calm, with a loving but dominant mother. He felt a strong emotional bond with her, almost as if replacing the father’s role. He became less pessimistic and more active, starting to engage in small daily activities. Over time, his energy increased significantly (to around 70–80%). He returned to work, established more routine, and incorporated activities such as walking, playing music, and yoga.
His condition stabilized progressively over several months. Although some fatigue remains (partly linked to medication and later also sleep apnea), his overall functioning improved markedly. He is working full-time, maintains daily structure, and reports a stable and satisfactory quality of life while continuing conventional treatment. He significantly decreased the medication, however does not dare to stop them, but he is satisfied with his life.
